New Patient Appointments
To request an appointment with On Point Integrated, new patients may call our office at 206-518-7800 or request an appointment through Zocdoc. After requesting an appointment, patients must complete our secure online intake form and submit a valid government-issued photo ID and current insurance information, if applicable.
Appointments are scheduled only after all required information is received and verified. Submitting a request through Zocdoc or by phone does not confirm an appointment.

Self-Pay Patients
If you do not have health insurance, do not wish to use your insurance, or have inactive, out-of-network, or unverifiable insurance, you may qualify for our self-pay pathway. Self-pay patients must provide complete demographics and a valid government-issued photo ID before an appointment is confirmed. You will be asked to acknowledge that you are responsible for all charges not covered by insurance and agree to pay the applicable self-pay fee before or at the time of service.
Required Acknowledgments
Before scheduling, all patients must confirm that the information submitted is accurate, authorize the practice to verify insurance eligibility and benefits, and understand that the appointment is not confirmed until the practice reviews and verifies all information.
Secure Intake
All information submitted through our website is collected and transmitted using a secure, HIPAA-compliant method. We do not send uploaded ID images, insurance images, or form responses through standard email.
Confirmation
After you submit your request, our staff will review your information and contact you regarding appointment confirmation. For insurance patients, we will verify your identity and coverage. For self-pay patients, we will confirm the current self-pay fee and payment requirements. Submission of the form does not confirm an appointment.
Sliding Scale Rates:
30 minutes: $150
45 minutes: $200
50 minutes: $225
60 minutes: $275
75 minutes: $325
90 minutes: $375
Sliding scale fees are determined based on visit length, clinical complexity, and individual patient circumstances. Please contact our office to discuss eligibility, availability, and scheduling.
